Winter Sports Physicals/Re-Certifications

If you/your child is playing basketball, bowling, or swimming at Serra this winter, please see below.

Cheerleaders due not need to be recertified for basketball cheerleading if they cheered for football.  New cheerleaders, please see below.

WINTER SPORT PHYSICALS/RE-CERTIFICATIONS  are due to Ms. DeSalvo or to the main office by noon on Monday November 13th.

ALL PHYSICALS MUST BE DONE OR CERTIFIED ON OR AFTER JUNE 1, 2017 and be on the following PIAA paperwork:

Option 1:  IF YOUR CHILD DID NOT PLAY A FALL SPORT AT SERRA (football, girls'/boys' soccer, volleyball, cross country, golf, cheer), you need to fill out the entire 9 page packet:

Instructions are on the 1st page, previous to the needed 9 pages.

Option 2:  IF YOUR CHILD PLAYED A FALL SPORT AT SERRA and was not sick, injured, etc, just fill out:
(No medical professional is involved in this one!)

*This form is also used if your child was sick/injured and already turned in clearance paperwork and returned to their fall sport before it was over.

Option 3:  ALSO, IF YOUR CHILD DID NOT PLAY ANYTHING AT SERRA THIS FALL,  BUT ALREADY TURNED IN THE PHYSICAL PACKET EARLY, please fill out:  https://sites.google.com/a/serrahs.org/serra-catholic/my-learning-style
to make sure that your paperwork is up to date.

Option 4:  IF YOUR CHILD PLAYED A FALL  SPORT AT SERRA AND WAS SICK, INJURED, ETC and did not return to their fall sport before it was over, have the medical provider that took care of them fill out:

if they have already written their clearance on the medical provider's prescription paper, just staple it to the form linked above.

 Hard copies of the full packet are also available in Serra's main office.

Athletes CANNOT participate as of Friday November 17 without the above mentioned paperwork filled out correctly and completely.
